Steps to Initiate and Complete your Evaluation
1.) Email Samantha Heim, PMHNP at samanthapmhnp@gmail.com to inquire about scheduling a psychological evaluation. Certified interpreter, Ms. Vanusa Sobrinho, M.Ed., will service your interpreting/translation needs, if required. If Ms. Sobrinho is booked for services, she will require a $100 deposit.
2.) Work with Ms. Heim to schedule your appointment on any weekday or weekend, morning, afternoon, or evening, depending on availability. You will receive a mental health questionnaire via email.
3.) Complete your questionnaire and email your responses back to samanthapmhnp@gmail.com at least two days before your scheduled appointment so that Ms. Heim may review your answers. If your answers are written in Portuguese, please submit them at least 5 days before your appointment.
4.) You will have up until the date of your appointment to complete a $500 deposit. Venmo and Zelle payment accepted. The balance remaining to Ms. Heim and Ms. Sobrinho (should her service be required) will be paid within three weeks following the appointment date and before your final evaluation report is released to you. Cash payments to Ms. Heim are accepted at a $100 discount. Please schedule a time and deliver your cash payment to 1395 Atwood Ave, Suite 201, Johnston, RI 02919.
5.) Meet for your virtual appointment at the scheduled time. Follow the link provided in your email and meet virtually, typing in your name to 'enter the waiting room'. Samantha Heim, PMHNP will click on your video to begin appointment.
6.) Within three weeks following your appointment, pay your remaining balance, and receive your psychological evaluation in English and/or Portuguese language. If you required translation services, you will pay the balance of $300-400 to Ms. Vanusa Sobrinho, M.Ed prior to receiving your evaluation (Venmo/Zelle).
7.) Other costs may be associated with your case, which will be discussed prior to you consent to evaluation. Other costs may include additional work around: Affidavit, court testimony, and complex case work.
