Fees for Services
Psychological Evaluation
The bulk of your evaluation will occur during your virtual appointment with Samantha Heim, PMHNP. Ms. Heim will also review your mental health questionnaire responses and collateral documents, which contribute greatly to your evaluation. The total cost is $800.
Collateral Document Review
Ms. Heim will review a reasonable amount of collateral documents (such as previous evaluations or medical/mental health records). If it is more time-consuming than usual, the extra fee will be $150, to be discussed before review and charge.
Written Affidavit
Some clients require a written testimony for court. Samantha Heim will create this legal document for you, as required. The extra fee for this document is $250.
Interpretation During Appointment
The cost for interpretation during your evaluation with Ms. Heim, will be $100. Ms. Vanusa Sobrinho, M.Ed, certified interpreter/translator, will be present in the room with Ms. Heim, providing interpretation to Portuguese.
Court Testimony
Some clients require that Ms. Heim present in court to provide sworn testimony. Should you require this, the fee will be $150 per hour in court, and $100 per hour for the travel time.
Written Translation
Ms. Sobrinho will provide written translation of your mental health questionnaire responses prior to your evaluation appointment with Ms. Heim. This fee will be $150. Ms. Sobrinho will also translate your final evaluation report to Portuguese. This fee will be $150.
